Prescription Medicine and Food Refill Form

Please submit your completed request form and we will do our best to have the items ready for you to pick up within 48 hours of receipt (excluding non-business days).  A staff member will contact you by e-mail once your order is ready, or if there are any questions/issues regarding the request.  We will also provide you with the invoice total and you may either drop off a check when picking up the items (PREFERRED PAYMENT METHOD), or provide us with credit card information.
